Interest Form Privacy Notice

This notice covers the EEE Travel Nurse Leadership Accelerator interest form. Contact Dr. Tanya Scott Cannady at dr@tanyascottcannady.com with privacy questions or requests.

Information and purpose

We collect the name, email, optional phone number, program preferences, leadership interests, and email permission you submit, along with submission metadata provided by our form service. We use these details to respond to your inquiry and manage program interest. Promotional email is optional. Providing a phone number does not authorize marketing texts.

Service providers

The form sends information through FormSubmit to our business email inbox. FormSubmit and its spam-protection services may process technical information to deliver submissions and prevent abuse. See FormSubmit’s privacy terms. The website hosting and email providers also process information necessary to operate their services. We do not sell interest-form submissions or disclose them to other businesses for their own marketing.

Your choices

You may leave optional fields blank or contact us directly instead of using the form. Email us to request access, correction, deletion, or removal from program updates. Each promotional email should provide an unsubscribe method. We may retain limited records needed to honor opt-outs or legal obligations.

Retention and security

We retain inquiries while needed for program follow-up and applicable recordkeeping. Contact us to request deletion. FormSubmit states it retains submission archives for 30 days. HTTPS protects transport, but no online service can guarantee absolute security. Do not submit patient information, medical details, passwords, or financial information.

Tracking and changes

This form does not add advertising pixels or marketing cookies. Third-party delivery and anti-spam services may use their own technical identifiers. This site does not change its form behavior in response to browser Do Not Track signals. We will update this notice when form practices change and display the revised effective date.
